The double dilemma of compliance and questionable efficacy of silicone products.

Silicone gel sheeting or cushions and pressure garments are difficult and troublesome, which makes compliance very low not only on exposed areas such as the face where it is not aesthetic.

Regardless of the indication, habitual use of a specific product is required to ensure results. So many people fail to complete or follow proposed usage for over-the counter (OTC) solutions, as well as prescription products, as a rule.

They found thirteen trials with 559 patients, 3 trials were prevention studies, eight trials looked for a treatment and two trials looked at both prevention and treatment strategies. Their conclusion: " Most studies were of poor quality and it is unclear whether silicon gel sheeting helps".

Another study, published in 2006, in Archives of Facial Plastic Surgery: official publication for the American Academy of Facial Plastic and Reconstructive Surgery, Inc. and the International Federation of Facial Plastic Surgery Societies, concludes: "Most treatments for keloidal and hypertrophic scarring offer minimal likelihood of improvement."

Topical silicone gel sheeting and intralesional steroids are the only evidence-based recommendable forms of treatment to control the quality of a scar. There are several studies about the advantages and disadvantages of both. This study was undertaken to verify the efficacy of a new topical silicone treatment: a self-drying spreadable gel that needs no means of fixation and cannot be seen because of complete transparency. Fresh surgical scars treated with the tested product showed significantly better outcomes than those untreated in a prospective trial involving a group of 160 patients. Patient compliance was particularly good, especially for scars on exposed areas such as the face, where the traditional gel sheeting is frequently discontinued at an early stage by patients who object to its visibility.

The results of the self-drying silicone gel have indeed been satisfactory. Considering the effective results obtained and the good patient compliance, the authors currently rate this concept of treatment as the first choice for preventing hypertrophy of recent scars.
